Non-Discrimination
You will not be denied appropriate care on the basis of race, religion, creed, color, national origin, sex, age, disability, handicap, marital status, sexual preference or source of payment.

 

Adequate and Appropriate Care
You are entitled to receive adequate and appropriate care.

Information and Participation in Care
You are entitled to know who is responsible for and who is providing your direct care; to receive information concerning your continuing health needs and alternatives for meeting those needs; to be involved in decisions about your care; and to be involved in your discharge planning, if appropriate.

Informed Consent
You are entitled to receive, from the appropriate individual within the hospital, information about your medical condition, proposed course of treatment, and prospects for your recovery, in terms that you can understand, unless medically contraindicated as documented by the attending physician in your medical record.
Treatment Choices and Refusal of Treatment
You are entitled to participate in decisions regarding your care, to refuse treatment to the extent provided by law, and to be informed of the consequences of your refusal. (Note: when your refusal of treatment prevents a hospital or its staff from providing appropriate care to you, according to ethical and professional standards, the relationship with you may be terminated upon reasonable notice.)
Experimental Procedures
You are entitled to information concerning an experimental procedure proposed as part of your care, and have the right to refuse to participate in the experiment without jeopardizing your continuing care.
Privacy
You are entitled to privacy, to the extent feasible, in treatment and in caring for your personal needs with consideration, respect, and full recognition of your dignity and individuality.

Confidentiality
You are entitled to confidential treatment of personal and medical records, and to refuse their release to a person outside the hospital without your prior authorization, except as required because of transfer to another health care facility or as required by law or because of third party payment contract.
Access to Records
You are entitled upon request to inspect or receive, for a reasonable fee, a copy of your medical record, but a third party shall not be given a copy of your medical record without your prior authorization.
Consultation on Social Service Needs
You and your family and other surrogate decision makers are entitled to request the advice and counsel of the hospital social services department as to the availability of other health care services and community services that may be helpful in your situation.
Spiritual and Pastoral Care Consultation
You and your family and other surrogate decision makers are entitled to request the advice of the hospital's pastoral services department in order to assist you in accessing religious and spiritual advisors who will be respectful and considerate of your personal values, beliefs and spiritual needs.
Patient Advocates and Advance Directives
You are entitled to designate a patient advocate as your surrogate in case you are unable to participate in making your own medical decisions, and you are entitled to give advance directives, which are instructions about your desires for care, custody and medical treatment when you are unable to participate in your own medical treatment decisions.
Pain
You are entitled to:
  • Be believed when you say you have pain.
  • Have your pain assessed on an individual basis using appropriate pain scales.
  • Have your pain and medication history taken.
  • Be provided with information about pain management and pain relief.
  • Be included in decisions about managing your pain.
  • Be included in developing a pain plan with your caregivers for management of your pain.
  • Know what medication, treatment or anesthesia will be given and the risks, benefits and alternatives.
  • Appropriate treatment of your pain.
  • A concerned and committed staff that responds quickly to reported pain.
  • Have your pain assessed at regular intervals.
  • Include your family in decision making, if desired.
  • Dedicated pain relief specialists if needed.
Ethics Committee Consultation
You and your family and other surrogate decision makers are entitled to request the advice and counsel of the hospital ethics committee on ethical issues concerning your treatment options
Access to Protective Services
You are entitled to assistance from hospital staff in accessing the county office of adult protective services in any situation in which there is a danger of abuse or harm by any other person, including any hospital personnel.
Billing Information
You are entitled to receive and examine an explanation of your bill regardless of the source of payment and to receive, upon request, information relating to financial assistance available through the hospital.
Insurance Information
You are entitled to receive assistance from the hospital billing department in understanding your insurance coverage or other third party payor coverage, and any exclusions or limitations.
Complaint Resolution
You are entitled to exercise your rights as a patient, resident and as a citizen; to receive information about the hospital's policies and procedures for initiation, review, and resolution of patient or resident complaints; and to present grievances or recommend changes in policies and services on behalf of yourself or others to your physician, hospital staff, to hospital administration or the compliance officer, or to another person of your choice within or outside the hospital free from restraint, interference, coercion, discrimination, or reprisal.
Right to Associate with Others
You are entitled to associate and have private communications and consultations with your physician, attorney, or any other person of your choice and to send and receive personal mail unopened on the same day it is received at the hospital, unless medically contraindicated as documented by your attending physician in your medical record. Your civil and religious liberties, including the right to independent personal decisions and the right to knowledge of available choices, shall not be infringed and the hospital shall encourage and assist you in the fullest possible exercise of these rights. You may meet with, and participate in, the activities of social, religious, and community groups at your discretion, unless medical contraindicated as documented by your attending physician in the medical record.
Freedom from Abuse and Restraints
You are entitled to be free from mental and physical abuse and from physical and chemical restraints. To protect your health and safety, restraints may be needed after alternatives have been tried and failed. The least restrictive restraint will be used for the shortest possible time as determined by your care provider. In an emergency arises and you need protection to prevent injury to yourself or others, a qualified professional may apply a restraint and then promptly contact your physician to obtain authorization to continue the restraint.
Freedom from Performing Services
You are entitled to be free from performing services for the hospital that are not included for therapeutic purposes in the plan of care.

You are responsible to…

 

  • Provide complete and accurate information to your caregivers, including your medical history, pain and pain relief methods.
  • Ask questions about your care, pain and pain management, what you are expected to do, or if you do not understand what you have been told.
  • Follow the recommendations of your physician, or tell your care provider if you are concerned or not able to follow them.
  • Understand and accept the outcome and consequences of deciding that you cannot follow the advice of your physician.
  • Tell your care provider if you have an unexpected change in condition, side effects from medication, your pain is not relieved, or you feel that your care is not going the way you think it should.
  • Follow the OHS rules and regulations, including the smoking policy, that apply to your care and conduct.
  • Respect other patients, OHS employees and property and be considerate by helping control noise and disturbances.
  • Give OHS accurate information about sources of payment for your bill.
  • Pay your part of the bill in a timely manner.
  • Give us suggestions about your needs and expectations and how we can make care better and safer.
