Instructions to Access Orientation Information:
Please review the nursing clinical placement definitions below and click on the title that describes your clinical assignment. A page listing all of the orientation documents you need for your clinical placement will appear. You may review the orientation information from the web site or print and review the paper version of the information. All information, including forms, can be printed and copied by you as needed for your clinical experience.
| Those placements where a faculty member and a student group are assigned to an area. Faculty remains on site with the students throughout the clinical experience. |
| Those placements where a faculty member is a coordinator for clinical placement for an NP or MSN candidate. The student is precepted by an Oakwood staff member. Faculty do not remain on site with the student throughout the clinical experience. |
| Those placements where a faculty member is a coordinator for clinical placement for a senior nurse. The student is precepted by an Oakwood staff nurse. Faculty do not remain on site with the student throughout the clinical experience. |
| Those placements where a faculty member is a coordinator for clinical placement for a senior nurse. The student is mentored by an Oakwood nurse manager. Faculty do not remain on site with the student throughout the clinical experience. |
| Those placements where a faculty member is a coordinator for clinical placement for a vocational high school student. Faculty may or may not remain on site with the student throughout the clinical experience. |
| Those placements where a faculty member is a coordinator for clinical placement for a vocational high school student. Faculty may or may not remain on site with the student throughout the clinical experience. |
| Those student NPs, MSN students, student nurses, student Medical Assistants placed in any Ambulatory Division clinical site. Please also click here if you are an agency person or volunteer working in the Ambulatory Division. |
| Please click here if you are an EMT, Paramedic, or other type of PreHospital Care Provider. Faculty do not remain on site throughout clinical experience. |

Orientation Instructions:
Please review the documents for your clinical placement. As faculty, you are responsible to ensure your students are knowledgeable of the orientation information. Please pay particular attention to the Student Clinical Placements Policy, which contains your student/faculty guidelines for clinical placements. Please be sure to contact the manager of the area you will be assigned for your clinical placement as soon as possible in order to arrange a date and time for your unit orientation.
Here's a review of what you need to turn in to your nursing clinical site coordinator prior to the start of your clinical experience. Your clinical experience cannot begin until all required paperwork is received by Oakwood.
Schools NOT PARTICIPATING in ACE
Please complete and turn in all seven items listed below.
- Copy of Your Course Objectives
- Faculty/Student Health Information Form: If you as faculty will be on site with the student(s), then your health information must also be included on the form.
- Unit Faculty Orientation Form: Only required for faculty that will be on site with their students.
- Student Orientation Checklist
- Nonemployee Data Access Form: This is needed for OakChart password obtainment for on site faculty and each student.
- Answer Sheet for Infection Control Module and Principals & Practice of Environmental Safety & Emergency Response Module: If you, as faculty, will be on site with your students, then only you need to complete these modules and tests. If you will not be on site with your students, then your student(s) need to complete these two modules and tests. You/your student(s) as appropriate must successfully pass these tests with a score of 90% or greater. Remediation will occur if a score of less than 90% is obtained.
- Accudose System Access Request Form: This Accudose document is ONLY required for FACULTY coming to Oakwood Hospital & Medical Center in Dearborn.
Schools PARTICIPATING in ACE
