Oakflex
Benefit program includes Medical, Dental, Vision, Life Insurance, and Flexible Spending Accounts. All Oakflex elections are effective the first of the month following 90 days employment. Insurance choice changes occur once/year unless life changing incident (ex. Marriage, divorce, birth, death, main provider unemployed).
Medical election available the first of the month following date of hire.
Holidays
8 paid holidays per year: New Year's Day, Memorial Day, Independence Day, Labor Day, Thanksgiving Day, Christmas Eve Day, Christmas Day (eligible from date of hire), Floating Holiday (eligible after one year of service).
Combine Time Off (CTO)
Accrual based on years of service: 0-4 years (.076924 per paid hr), 5 -9 years (.096154 per paid hr), 10 + years (.115385 per paid hr). Eligible after 90 days of continuous employment. Payoff at termination if emplyed one year.
Life Insurance and AD&D
Amount equal to 2080 hours times base rate of pay. Effective the first of the month following 90 days of employment.
Short Term Disability
Basic Plan:
14 day waiting period for illness, accident, or hospitalization; must use CTO bank. After waiting period, receive 60% of Weekly base earnings (40 hours x base rate of pay) to maximum of 24 weeks. Eligible after 6 months of employment.
Enhanced Plan:
Same as Basic Plan except there is no waiting period for disability due to accident, hospitalization or surgery.
Long Term Disability
60% of monthly base earnings with $4,000 monthly maximum at no cost! Elimination period of 180 days consecutive disability. Eligible after 6 months of full time employment.
Retirement
Cash Balance Plan with no employee contribution required. Vested in 5 years. Employer contribution based on employee's years of service and base earnings.
Tax Deferred Annuities
403(b) savings program through payroll deductions that defers Federal and State taxes. Employer will make a matching contribution of 50% on the first 4% of base pay. Employer match vested in 5 years.
Tuition Reimbursement
100% of approved tuition. Maximum $1,200/year. Eligible after 6 months employment.
Direct Deposit
Available with all banks and credit unions in the USA and Canada (Royal Bank of Canada).
