Medical Records Release - Before Oakwood Healthcare will release your medical records to a third party, you will need to fill out this form giving your consent.

Parental Consent Form- When your child needs to see the doctor, but you cannot be there, fill out this form and send it with the adult who is accompanying your child. The doctor needs parental consent to treat your child.

School/Sports Physical Form- You will need to fill out this form in order for your child to be given his or her physical before participating in sports.

Immunization Schedule- An immunization schedule and form for you to keep with your records.

Child Medical History - The Child Medical History form provides the physician with the current and past health history of your child. This information is important for the physician to know as they provide the ongoing care of your child.

Adult Medical History - The Adult Medical History form provides the physician with your current and past health history. This information is important for the physician to know as they provide your ongoing care.
