Call office between 9:00 am and 5:00 pm Monday through Friday. If office is closed, follow instructions on the answering machine.
Office Policies
Payments, including co-pays, co-insurances, deductibles and self-pay, are expected at time of service. For your convenience, cash, checks and credit/debit cards are accepted.
