Patients are responsible for providing accurate and up to date insurance information. The practice accepts cash, credit cards. and check. Co-payments are excepted on the day of service. Payment plans and discounts are available for patients without insurance or large deductibles.
On the first visit patients should arrive 15 minutes before their scheduled appointment time which allows for completion of office paperwork. Please bring your insurance cards and a list of your medications. Questions and concerns about your health are encouraged when you see the physician.
