After Hours Instructions
I offer extended hours on Mondays between 7:30 am- 6:30pm. After hours and on weekends patients can call the answering service to be connected with the on-call physician.
Office Policies
Payments, including co-pays, co-insurances, deductibles and self-pay, are expected at time of service. For your convenience, cash, checks and credit/debit cards are accepted.
What To Expect
New patients are asked to arrive 30 minutes prior to their scheduled appointment time. Patients should bring their current insurance card and drivers license.
