    Accept majority of insurances. Copayment and other dues are expected at the time of services. Please bring referral form from your PCP if you have HMO or bring a Claim number if involved in an accident.

    What To Expect

    Bring old relevant medical records as well as a note from referring physician about reason for consultation. Please make your list for neurological problems.  If your insurance requires a referral from your primary care physician please bring the required referral to your first visit with us.
