Office Policies
Copays are due at the the time of the visit. A payment plan can be worked out for cash paying patients. We also accept payment by VISA or MASTERCARD.
What To Expect
Bring ALL of your medications with you on your first visit, including herbal and over the counter products. Bring any other medical records you have, as well as the address and telephone number of your last primary care physician.
