Monday/Wednesday 8a.m.-5p.m.
Tuesday/Thursday 9a.m.-5p.m.
Friday 9a.m.-4p.m.
Answering service will answer all after hour calls and page the doctor.
Office Policies
Co pays are due at the time of service, unless other arrangements have been made in advance. Outside billing service will bill patient balances directly to the patient
What To Expect
Please bring drivers license and insurance card(s). Also any test results that may be related to the reason for your visit.
