Office Policies
We do not accept personal checks or credit cards on a first visit. However, we do accept all forms of payments at our facility thereafter.
What To Expect
You need to bring the following pieces of information with you. You have to be absolutely sure that it is all current.
1) Medication: Please bring all the meds you are currently taking.
2) ID: Please bring your ID.
3) Insurance: You also need to bring your current insurance information.
