Self Management

  • Oakwood is committed to providing you with the best diabetes care possible. Our clinical program is approved by the American Diabetes Association, therefore you know you will be getting the best training available. We work with your primary care provider to plan diabetes care and educational services that will help you to learn to manage your diabetes and control your blood sugar levels. And, controlling blood sugar can prevent the complications of diabetes.
  • Our services are covered by most insurance plans and an order is required from your doctor.
  • We meet with you individually to address your concerns privately and assist in setting personal goals. We also provide group classes where you will learn from others with diabetes as well as the instructors.

  • Medical Nutrition Therapy (MNT)
  • MNT provides individualized counseling with a registered dietician. This benefit is covered Medicare for those with diabetes and/or kidney disease. Many private health insurance providers cover MNT as well. Check with your insurance plan to confirm MNT coverage. You may be referred at any time by yourself, your doctor, or other healthcare provider. You will need a prescription or referral requesting diabetes self-management education and/or medical nutrition therapy from your doctor if you desire insurance coverage for services.
