Request an Application for Clinical Privileges within the Oakwood Healthcare System

  Initial Application Request
Thank you for your interest in joining the affiliated hospitals of Oakwood Healthcare System. Oakwood is a leader among healthcare systems in the Southeastern Michigan area. Each Oakwood affiliated hospital has its own medical staff. However, you can apply to any and all hospitals in the system through completion of one application; membership and privileges at each facility will be considered separately. Staff Status Definitions
  • Active – regularly admit patients to or are involved in care of patients in the hospital
  • Courtesy – occasionally admit patients to or care for patients in the hospital, but who do not otherwise participate actively in hospital work
  • Consulting – recognized experts in their respective field [no admitting privileges]
  • Ambulatory – do not personally admit or care for patients in the hospital whose practice is affiliated with an OHS outpatient facility
  • Visiting – do not personally admit or care for patients in the hospital, in private practice
  • Affiliate - non-physician providers including physician assistants, nurse practitioners, nurse midwives, physician-employed registered nurses; and physicians who provide moonlighting services
All applicants to the OHS staff must possess an unrestricted State of Michigan Medical and Controlled Substance license, Federal DEA, malpractice insurance with limits of at least $100,000/$300,000 and be board certified in their practicing specialty (unless a recent grad who has not had the opportunity to sit for the boards)

Please follow the link above to download and complete the Initial Application Request Form and return it to the Central Verification Office (CVO) via email to emailapps@oakwood.org. Please provide a current email address on the application; the CVO will notify you, through that email address, if you meet the application requirements.

The entire initial credentialing process will take approximately 3-4 months; please plan accordingly. Thank you again for expressing an interest in joining the Oakwood Healthcare System family!
