Payments, including co-pays, co-insurances, deductibles and self-pay, are expected at time of service. For your convenience, cash, checks and credit/debit cards are accepted.
Please bring your medications and/or a list of your medications including over the counter supplements. Also, please bring a list of questions. If there are reports you would like to review with me, please bring to your appointment.
