- Complete all highlighted areas. Be sure to specify the dates of service and type of information needed (i.e., ER report from 6/10/06 visit).
- Place the completed authorization form in an envelope and mail to Medical Records at the appropriate location below.
If you are requesting records sent to you or to a physician outside of Oakwood, you will receive a bill. If you have any questions, do not hesitate to call us at the one of the phone numbers listed below.
Requesting Medical Records of a deceased patient
Please contact the Medical Records Department at the appropriate location. You may be asked to provide additional documents to obtain a copy of the records.
